UMIA Arbitration Program
UMIA provides copies of the approved arbitration agreement along with a video tape in English and Spanish and written materials to explain the program to the patient. These materials are suitable for reproduction.
The following are steps to implement the program:

1. Obtain a packet of arbitration materials from
UMIA.
2. Order printed materials from Paramount Press, (801)461-0500
3. Organize and train your office staff about arbitration and how to present it properly.
4. Provide the written explanation and agreement to the patient when first coming to the practice or on their next visit if they have not already signed an agreement.
5. Have a designated and trained staff member ask if they have any questions about the agreement, and then ask them to sign the agreement.

a. If the patient signs the agreement, it must be signed
and dated by the physician.
b. If the patient refuses to sign the agreement, the staff member should notify the physician so it may be addressed by the doctor directly with the patient.
c. Refusals should be documented in the chart.
d. Signed agreements should be securely fastened in the patient’s chart.
6. The patient may revoke the agreement in writing within 10 days
7. The agreement automatically renews on its anniversary if not revoked 10 days prior in writing.
