ISMP Tool to Assess High-Alert Medication Risk

The Institute for Safe Medication Practices (ISMP) has released a new tool, ISMP Medication Safety Self Assessment® for High-Alert Medications, offering hospitals, LTC facilities, and outpatient organizations the opportunity to assess the safety of systems and practices associated with high-alert medications. High-alert medications are those drugs that bear a heightened risk of causing significant harm when used in error.

ISMP encourages facilities to submit their self-assessment findings to ISMP online to obtain weighted scores. Participating organizations will be able to compare their results with other demographically similar organizations.
